Guidelines for Application to the MRB

In your cover letter, please provide all of the following information:
- Contact information, including mailing address, phone number(s), and email address.
- Are you a U.S. citizen?
- Are you a Federal employee?
- Type of medical license (i.e., M.D. or D.O.), State(s) of licensure, license number, and current standing of your license.
- What board certification(s) do you have?
- What advanced degrees do you hold? (e.g., M.P.H., Ph.D.)
- In which transportation safety or occupational medicine organization(s) do you participate? (e.g., NCS, ACOEM, MROCC)
- What is your level of participation in these organizations? (e.g., general member, board member, chairman)
- With which transportation safety or medical professional organization(s) have you held a leadership role? (e.g., AMA, AOA, ACP)
- What leadership role do/did you hold?
- How many standards and/or guidelines have you developed related to general medical topics?
- How many medical standards and/or guidelines have you developed related to transportation safety?
- How many of these standards or guidelines were developed in a local, State, or Federal regulatory environment? Please indicate which agencies.
- Have you published research or scientific investigations in peer-reviewed publications? If so, how many? What is/was your role?
